Small Claims Forms

Some forms used at Court are considered attachments to the Local Rules. Since Small Claims Local Rules are included with the Civil Local Rules, some Small Claims forms are on Civil Local Rules attachments page.

For example, forms SC-8006 (Notice to Small Claims Litigants) and SC-8007 (Request for Dismissal) are listed on the Local Civil Rules of Court Attachments page of this website.

Small Claims forms that are not part of the Local Rules can be found on the Superior Court Local Forms page, which lists all local forms for this court.  Small claims form numbers all begin with the letters "SC" - for example, SC-8010 (Request and Order to Serve the California Secretary of State) and SC-8011 (Request for Satisfaction of Judgment by Judgment Debtor) can be found on the Superior Court Local Forms page.
